If anaerobic organisms are suspected of contributing to the infection, appropriate therapy should be administered. Appropriate culture andsusceptibility tests should be performed before treatment in order to isolate and identify organisms causing infection and to determine theirsusceptibility to ciprofloxacin. Therapy with Cipro IV may be initiated before results of these tests are known; once results become availableappropriate therapy should be continued. As with other drugs, some isolates of Pseudomonas aeruginosamay develop resistance fairly rapidly duringtreatment with ciprofloxacin. Culture and susceptibility testing performed periodically during therapy will provide information not only on thetherapeutic effect of the antimicrobial agent but also on the possible emergence of bacterial resistance.

Unfortunately, CSA has also been understood only as penetrative sex in this section. The fact that CSA can take forms where contact or touch is notrequired (exposing or made to expose genitalia, showing pornography etc.) or where there is no penetration falls outside of the ambit of this section.Non-penetrative and non-touch CSA gets covered under Secs. 354 and 509 of the IPC (outraging the modesty of a woman, which only includes the girlchild), though these sections remain woefully steeped in the discourse of female honour. Ideally, it should be included in a graded fashion under thesexual assault section in the CLA. The CLA also does not include incest and the processes of grooming that precede sexual contact in any case of CSA.The tokenistic insertion of this provision in the CLA does great disservice to the demand of child rights groups and the Sakshi petition for aseparate and dedicated criminal law on CSA. Though the CLA gives considerable attention to punitive measures by increasing punishments and creatingnew crimes, a glaring omission is the absence of any provision for children with disabilities whose vulnerability to sexual abuse may be highercompared to other children.

Although effective in clinical trials, ciprofloxacin is not a drug of first choice in the pediatric population due to an increased incidence ofadverse events compared to controls, including events related to joints and/or surrounding tissues. Cipro IV, like other fluoroquinolones, isassociated with arthropathy and histopathological changes in weight-bearing joints of juvenile animals [see Warnings and Precautions (5.11), AdverseReactions (6.1), Use in Specific Populations (8.4), and Nonclinical Toxicology (13.2)].
Thus we have the CLA, coming over half a decade after the judgment, although CSA has been given an infantile short shrift. In the CLA, ‘rape’ has beenredefined as ‘sexual assault’ and includes penetration of any orifice on a woman’s body by any part of the man’s body or any other object. Consentremains the guiding factor to decide what qualifies as sexual assault. The age of consent is fixed at 18 years. However, “when penetration is carriedout for proper hygienic or medical purposes” it is not sexual assault – thus it allows for gross misuse as defense for medical personnel who can beperpetrators of CSA.
